Use of inhaled corticosteroids for the prevention and/or treatment of bronchopulmonary dysplasia in preterm infants: a systematic review protocol
نویسندگان
چکیده
BACKGROUND Inhaled steroids have been studied for both prevention and treatment of bronchopulmonary dysplasia (BPD). Results have been inconsistent. Recently, a large randomized controlled trial (RCT) has been reported. METHODS/DESIGN We will perform a comprehensive systematic literature search for randomized and quasi-randomized controlled trials that studied the efficacy and safety of inhaled corticosteroids administered to preterm infants (22-36 weeks) for either the prevention or treatment of BPD diagnosed by both clinical and physiological outcome criteria. We will assess potential risk of bias for each RCT meeting our selection criteria using the Cochrane risk of bias tool for RCTs. The primary outcome of interest will be mortality or BPD or both at 28 days postnatal age or 36 weeks postmenstrual age. Pooled estimates will be calculated using RevMan software with a random effects model as primary analysis. We will assess the quality of evidence using the Grading of Recommendations Assessment, Development and Evaluation (GRADE) approach. DISCUSSION Meta-analytic estimates of eligible RCTs, potentially including a new large RCT, may significantly influence neonatal practice in the prevention and treatment of respiratory problems in preterm infants. SYSTEMATIC REVIEW REGISTRATION PROSPERO CRD42015019628.

Inhaled budesonide for the prevention of bronchopulmonary dysplasia.
Theoretically, administration of inhaled corticosteroids may allow for beneficial effects on the pulmonary system of infants with evolving or established bronchopulmonary dysplasia (BPD) with a lower risk of undesirable side effects compared to systemic corticosteroids. However, before deciding whether to use inhaled corticosteroids for BPD in routine clinical practice, the available randomized...
متن کاملComparing the Efficacy of High and Low Doses of Vitamin A in Prevention of Bronchopulmonary Dysplasia
Background Bronchopulmonary dysplasia (BPD) is one of the most common serious squeal of preterm infants. It involves approximately one quarter of infants with birth weight less than 1500 grams and 30% of less than 1000 grams. Vitamin A has been shown to reduce BPD rate. We compared efficacy of low and high doses of vitamin A for prevention of BPD in very low birth weight preterm infants. Materi...
متن کاملPolicy Statement—Postnatal Corticosteroids to Prevent or Treat Bronchopulmonary Dysplasia
The purpose of this revised statement is to review current information on the use of postnatal glucocorticoids to prevent or treat bronchopulmonary dysplasia in the preterm infant and to make updated recommendations regarding their use. High-dose dexamethasone (0.5 mg/kg per day) does not seem to confer additional therapeutic benefit over lower doses and is not recommended. Evidence is insuffic...
متن کاملPostnatal hydrocortisone for preventing or treating bronchopulmonary dysplasia in preterm infants: a systematic review.
BACKGROUND Corticosteroids have been used widely after birth in preterm infants with respiratory failure; hydrocortisone may be preferable to other corticosteroids for this purpose. OBJECTIVES To determine if postnatal hydrocortisone is useful to prevent or treat bronchopulmonary dysplasia (BPD) in preterm infants. METHODS Randomised controlled trials (RCTs) of postnatal hydrocortisone ther...
متن کاملCould mean platelet volume predict developing of bronchopulmonary dysplasia in preterm infants with respiratory distress syndrome?
Introduction: some studies have suggested correlation between MPV index and inflammatory diseases such as rheumatoid arthritis in adults. Though bronchopulmonary dysplasia is also an inflammatory disease which develops in preterm neonates with Respiratory distress syndrome, we decided to study the possible correlation between the mean platelet volume (MPV) and the occurrence of bronchopulmonary...
